29 CFR 1910.146(k)(2)(iv): The employer did not ensure that affected employees practice making permit space rescues at least once every 12 months, by means of simulated rescue operations in which they remove dummies, manikins, or actual persons from the actual permit spaces or from representative permit spaces:    (a)    On or prior to 6/17/15, throughout the facility:   Simulated permit space rescues were not practiced by employees tasked with rescuing entrants in permit required confined spaces such as tanks and vessels which are entered for routine cleaning, maintenance, and inspection.       On 6/1/15 through 6/8/15, employees conducted multiple entries of permit required confined spaces, including but not limited to:  UTM, Wire Pit, TAD 2, Yankee, Clear Well, and white water chest for purposes of cleaning, maintenance, and inspection.
