29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about May 19, 2015, at the excavation site located in Norman, OK, employees were observed working in a trench more than 4 feet deep that had no ladder or other safe means for employee access/egress as employees were working in the trench. Employees were exposed to the hazards of struck-by or crushed-by.

29 CFR 1926.652(e)(2)(i): Excavation of material to a level no greater than 2 feet (.61m) below the bottom of the members of the support system was allowed when there were indications while the trench was open of a possible loss of soil from behind or below the bottom of the support system: On or about May 19, 2015, at the excavation site located in Norman, OK, employees were observed working in a trench that had the trench box elevated more than 2 above the trench floor. Employees were exposed to the hazard of being struck by falling soil and equipment.
