On 05/02/2015, an employee fell from the unprotected edge while in the process of unloading drywall sustaining injuries and was hospitalized.

Adamo Drive, Tampa, FL 33605, fall protection training was not provided for employees who were exposed to fall hazards while delivering drywall to multi-story buildings. On 05/02/2015, an employee of Staffex fell 14 feet while unloading drywall on the second story of a building under construction and was hospitalized.

The extension ladder rails did not extend at least 3 ft. above the upper edge exposing employees to fall hazards of 14 ft.
