Adamo Drive, Tampa, FL 33605, as observed on or about 05/21/2015, training on ladder hazards was not given. Employees were not aware of ladder hazards or ladder safety procedures.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) For the construction site at 2810 N 35th St., Tampa, FL 33605, as occurring on or about 05/02/2015, employees were exposed to fall hazards of 14 ft. from unprotected floor edges on either side of a hallway being accessed. On 05/02/2015, an employee fell from the unprotected edge while in the process of unloading drywall sustaining injuries and was hospitalized.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    a) For Staffing Express employees working out of Gator Gypsum Tampa Main Yard at 3904 E. Adamo Drive, Tampa, FL 33605, fall protection training was not provided for employees who were exposed to fall hazards while delivering drywall to multi-story buildings. On 05/02/2015, an employee of Staffex fell 14 feet while unloading drywall on the second story of a building under construction and was hospitalized.

The extension ladder rails did not extend at least 3 ft. above the upper edge exposing employees to fall hazards of 14 ft.
