29 CFR 1910.178(l)(2)(i)(B): Trainees were allowed to operate a powered industrial truck where such operation endangered the trainee or other employees: On or about May 12, 2015, an employee lerning to operate a stand-up forklift was assigned to perform regular production tasks between loaded storage racks as part of the training. The employee's lack of proficiency in the operation of this foklift prior to the assignment created struck-by and caught in-between hazards to which the trainee and other employees were exposed. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). On or about May 12, 2015, the B.T. Prime Mover stand-up forklift, serial number RRX352810400, was not taken out of service when it was found to be in need of repair. The key was broken off in the ignition of the forklift resulting in employees having to use tools such as a box cutter to start and turn off the forklift. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
