On or about May 12, 2015, the B.T. Prime Mover stand-up forklift, serial number RRX352810400, was not taken out of service when it was found to be in need of repair. The key was broken off in the ignition of the forklift resulting in employees having to use tools such as a box cutter to start and turn off the forklift.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
