29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 05/28/15 in the Exhaust Hanger Department; employer failed to ensure the operator of the horizontal Muffler Hanger Winder applied a lock on the disconnect switch and the main air prior to performing service and maintenance on this machine, such as, changing of the spool and associated needle threading, as described in portions of the Muffler Hanger Winder SF 052 Energy Control procedure dated 11/30/2009, exposing them to puncture and caught between hazards. b) On or about 05/28/15 in the Exhaust Hanger Department; employer failed to ensure the operator of the vertical Muffler Hanger Machine applied a lock on the Machine Disconnect Switch and main air supply prior to performing service and maintenance on this machine, such as, changing the spool of winding and the associated needle threading, as described in the Muffler Hanger Machine SF052 Energy Control procedure dated 10/26/12, exposing them to puncture and caught between hazards. c) On or about 05/28/15 in the Exhaust Hanger Department; employer failed to ensure the operator of the Muffler Hanger Cutter applied a lock on the local disconnect switch and main air supply prior to performing service and maintenance on this machine, such as, changing the cutting blades, as described in the Muffler Hanger Cutter SF052 Energy Control procedure dated 11/30/09, exposing them to laceration hazards. ABATEMENT CERTIFICATION REQUIRED

The opening on guarding device was 7 inches wide and 11.25 inches from the point of operation ( cutting blades and rotating shaft ). 10) On or about 05/28/15 in the Textile Department; employer failed to protect employees who were assigned to operate the S&P Cutter from being able to access the point of operation (cutting blades) during operation when the diameter of the opening on the guarding device was 4.25 inches and the distance to the point of operation was 5.75 inches, exposing them to an amputation hazard. 11) On or about 05/28/15 in the Textile Department; employer failed to protect employees who were assigned to operate the S&P Cutter from being able to access the point of operation (cutting blades and the tension rollers) when the opening on the back side of the guarding device was 6 wide inches and the distance to the point of operation was 7 inches and 3 inches from the tension roller, exposing them to an amputation hazard. 12) On or about 05/28/15 in the Textile Department; employer failed to protect employees who were assigned to operate the stapler for the IBM Brush ( machine # 689358 - G2 ) from being able to access the point of operation (stapler head), when the diameter of the opening on the guarding device was 3 inches and the distance to the point of operation was 1.5 inches, exposing them to puncture hazards. ABATEMENT DOCUMENTATION REQUIRED Schlegel Systems Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.212 (a) (1) which was contained in OSHA inspection number 620798 Citation Number 2, Item Number 1 and was affirmed as a final order on May 10, 2013, with respect to a workplace located at 1555 Jefferson Road, Rochester, New York 14692.
