29 CFR 1926.100(a): At this job site, employees exposed to possible head injury from falling or flying objects or from impact with hand tools, equipment and building materials during construction operations were not wearing hard hats.   A). On 26 May 2015 at CORCO Construction site 4100 S. Campbell St. Springfield MO, employees were exposed to struck by hazards by not wearing hard hats.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as used as a means of access.    1). On or about 26 May 2015 at 4100 s. Campbell Street, Springfield MO, Employer CORCO failed to enforce proper access to mobile scaffolds exposing employees to fall hazard while employees were using crossbraces as means of access while the scaffold was extended more then 10 feet in the air.    2). Welders accessing the roof over the rail of the scaffolding a difference of 42 inches.

29 CFR 1926.501(b)(5): "Formwork and reinforcing steel." Each employee on the face of formwork or reinforcing steel shall be protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems.    1). On or about 26 May 2015 at 4100 s. Campbell Street, Springfield MO, Employer CORCO exposed its employees to fall from hazard. Two welders were on the top center portion of the building welding at various points one had no fall protection at all and the other was wearing fall protection but failed to connect it to an anchor point.
