29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about 11/22/2021, at the jobsite, where the elevator shaft opening on the second floor was not guarded on the north (interior) side, exposing employees to the hazard of falling approximately 21 feet into the elevator pit below the first floor.

29 CFR 1926.1052(c)(1)(ii):Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge: On or about 11/22/2021, at the jobsite, where employees ascending the north stairs on the west side of the building were exposed to the hazard of falling approximately 10-feet, 3-inches to 12-feet, 9-inches from the unguarded stairs through the window opening to the ground outside.
