29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by a pinch point between hydraulically operated machine parts: a) #1 Draw Block Machine, Coil Shop, 213 Vandale Drive, Houston, PA 15342: On or about May 21, 2015, a pinch point located between the cylinder mounting block plate and the back of the back pressure roll hydraulic cylinder was not guarded. Employees setting the cast for new orders were exposed to caught-between hazards. b) #2 Draw Block Machine, Coil Shop, 213 Vandale Drive, Houston, PA 15342: On or about May 21, 2015, a pinch point located between the cylinder mounting block plate and the back of the back pressure roll hydraulic cylinder was not guarded. 29 CFR 1910.219(c)(3): Vertical or inclined shafting seven (7) feet or less from floor or working platform was not enclosed with stationary casing(s): a) #1 Draw Block Machine, Coil Shop, 213 Vandale Drive, Houston, PA 15342: On or about May 27, 2015, the rotating cylinder of a vertical agitator was not enclosed with a stationary casing. Employees were exposed to caught-by hazards while filling the lubricant barrel and running coil through the lubricant barrel. b) #2 Draw Block Machine, Coil Shop, 213 Vandale Drive, Houston, PA 15342: On or about May 27, 2015, the rotating cylinder of a vertical agitator was not enclosed with a stationary casing.
