29 CFR  1910.95(d)(1): A representative monitoring program was not developed and implemented when information indicated that an employee's exposure may equal or exceed an 8 hour time weighted average of 85dBA:  a)  In the Facility, on or about May 17, 2022 - The employer did not develop and implement a representative noise monitoring program when information indicated that an employee's noise exposure may equal or exceed an 8 hour time weighted average exposure of 85 dBA.  Description of Employee Noise Exposures:  1.  An employee designated as a Grinder Operator was exposed to noise at a level of 82.8% of the allowable limit during a 450 minute sampling period on May 17, 2022.   This exposure is equivalent to a sound level of 88.6 dBA for the dose and time of employee exposure and is 1.66 times in excess of the 85 dBA action level (50%).  2.  An employee designated as a Grinder Operator was exposed to noise at a level of 88.9% of the allowable limit during a 448 minute sampling period on May 17, 2022.   This exposure is equivalent to a sound level of 89.2 dBA for the dose and time of employee exposure and is 1.78 times in excess of the 85 dBA action level (50%).  3.  An employee designated as a Grinder Operator was exposed to noise at a level of 96.3% of the allowable limit during a 447 minute sampling period on May 17, 2022.   This exposure is equivalent to a sound level of 89.7 dBA for the dose and time of employee exposure and is 1.93 times in excess of the 85 dBA action level (50%).  4.  An employee designated as a Grinder Operator was exposed to noise at a level of 117.3% of the allowable limit during a 450 minute sampling period on May 17, 2022.   This exposure is equivalent to a sound level of 91.2 dBA for the dose and time of employee exposure and is 2.35 times in excess of the 85 dBA action level (50%).
