29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On May 27, 2015, employees were not protected from electrocution hazards while working from an aerial lift in close proximity to 12.5 kV energized distribution lines and the employer did not ensure the lines were de-energized or effectively guarded. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.453(b)(2)(v): A body belt or harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated On May 27, 2014, employees were not protected from a 25 foot fall while operating an aerial lift and the employer did not ensure employees were utilizing appropriate fall protection. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
