Employees were exposed to struck-by hazards when driving loaded powered industrial vehicles (forklifts) through the potholes, as observed on or about May 27, 2015. In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous condition was corrected.

Employees were exposed to chemical exposure hazards to the eyes and face, on or about May 27, 2015. In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous condition was corrected.
