29 CFR 1910.22(b)(2): Permanent aisles and passageways were not appropriately marked. a) Southwest side, Transfer Terminal, 2382 S.R. 715, Tannersville, PA - Pedestrian passageway markings from the main office to the dock offices (across from docks 191 to 242) and pedestrian passageway markings near the exit door (between docks 206 and 207) had not been maintained and were no longer easily visible exposing employees to struck-by hazards from powered industrial trucks (forklifts), as observed on or about November 28, 2016.

29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides. a) The open-sided floor between the dockplates where employees entered/exited trailers was not guarded by a standard railing (or equivalent) exposing employees to a fall of four-foot, two-inches (4'2") onto asphalt, as observed on November 28, 2016.
