On or about May 26, 2015 - at the above addressed jobsite, the employer did not instruct the employees in the recognition and avoidance of unsafe conditions while working around the Cold Milling Machine (BOMAG FAYAT GROUP BM 1300/30), thereby exposing the employees to struck-by and/or crushing hazards.
