29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Employees working on pump jack scaffolding system without the benefit of fall protection were exposed to fall hazards of approximately 13 feet 6 inches. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end. East side of the scaffold platform extended approximately five feet past its support exposing employees to fall hazards of approximately 13 feet 6 inches. No abatement certification or documentation is required for this item.

29 CFR 1926.451(c)(2)(i): Footings shall be level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement. Footings for scaffold legs were not level and had the potential for settling or displacement. This caused the scaffold legs to be erected without bearing flush on the footing. No abatement certification or documentation is required for this item.
