29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2). Shaver Barge: On or about May 27, 2015 and times prior thereto, the port and starboard edges of the weather deck did not have guardrails or other means to protect employees. This exposed employees to approximately an 18 foot fall to the dry-dock floor.

29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches. Shaver Barge: On or about May 27, 2015 and times prior thereto, there were open flush manholes that were not guarded. This exposed employees to a fall of 15 feet to the lower level.

29 CFR 1915.56(b)(4): When welding cable became worn to the extent of exposing bare conductors, the portion exposed was not protected by means of rubber tape, friction tape, or other equivalent insulation. Weather deck Shaver barge: On or about May 27, 2015 and times prior thereto, welding cables were used that had cuts to the conductor. This exposed employees to potential electric shock hazard if they contacted conductor while welding.

29 CFR 1915.158(a)(1): PFDs (life preservers, life jackets, or work vests) were not worn by each affected employee must be United States Coast Guard (USCG) approved pursuant to 46 CFR part 160 (Type I, II, III, or V PFD) and marked for use as a work vest, for commercial use, or for use on vessels. USCG approval is pursuant to 46 CFR part 160, Coast Guard Lifesaving Equipment Specifications. Floating vessel: On or about May 27, 2015 and times prior thereto, employee was not wearing a PFD while working next to the water. This exposed employee to a potential fall into the water.
