A)     On or about June 14, 2021, employees were repositioning a 480 volt water purification system, while it was still powered, in the engine room of the tugboat "Aurora".  This caused an employee to receive an electric shock injury resulting in hospitalization. Failure to identify an energy source and use an energy-isolating device prior to servicing could result in serious injury or death.
