29 CFR 1915.89(c)(1):The employer did not ensure that, before any authorized employee performs servicing when energization or startup, or the release of hazardous energy, may occur, all energy sources are identified and isolated, and the machinery, equipment, or system is rendered inoperative. A) On or about June 14, 2021, employees were repositioning a 480 volt water purification system, while it was still powered, in the engine room of the tugboat "Aurora". This caused an employee to receive an electric shock injury resulting in hospitalization. Failure to identify an energy source and use an energy-isolating device prior to servicing could result in serious injury or death.
