29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specify procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: The employer does not ensure that energy control procedures clearly and specifically specify procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a) This violation occurred on or about May 20, 2015, located in the shop where an employee was exposed to caught-in and struck-by hazards when performing servicing tasks on the Controlled Automation Drill Line (SN: 05-057) without specific procedural steps for controlling hazardous energy when adjusting the coolant. b) This violation occurred on or about May 26, 2015, located in the shop where employees were exposed to caught-in and struck-by hazards when performing servicing tasks on equipment such as, but not limited to, a WF Wells bandsaw (SN: A2631/B05378) without specific procedural steps for controlling hazardous energy when changing the saw blade. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that energy control procedures clearly and specifically specify procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.

This violation occurred on or about May 20, 2015 and times thereafter, located in the shop where employees were exposed to caught-in and struck-by hazards when performing servicing tasks on equipment such as, but not limited to, a WF Wells bandsaw (SN: A2631/B05378) and a Controlled Automation Drill Line (SN: 05-057) without providing authorized employee training for servicing and/or maintenance tasks. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that authorized employees receive training.

Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that each authorized employee affix a personal lockout device to the group lockout before working on the equipment.
