29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a) Electrical Utility Room: An employee was trouble shooting electrical problems, using a volt meter on an energized Square D - Heavy Duty, Safety Switch (480V). Work practices were not consistent with the nature and extent of the associated electrical hazards on, or about, 05/29/2015. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: a) Electrical Utility Room: An employee was trouble shooting electrical problems, using a volt meter on a live Square D Heavy Duty, Safety Switch (480V). The activity initiated an electrical arc blast. The employee did not use protective equipment to prevent blast and burn injuries on, or about, 05/29/2015. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
