29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: a) Building 669, near the North overhead door. On or about 5-18-2015, employees were powering a Rigid portable pipe threading machine with an electric power cord that did not have a GFCI or was part of an assured grounding conductor program. Note: Abatement certification is required for this item.

On or about 5-18-2015, employees were using a Rigid portable pipe threading machine with an electric foot switch whose electric flexible cord was not secured into its strain relief device.
