29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:  a) On or about May 11, 2016 and at times prior thereto employees were exposed to fall hazards greater than 6 feet (6 feet 6 inches) at the mechanical room pump pit while accessing and installing plumbing materials, where no fall protection was available or used. 29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)  a) On or about May 11, 2016 and at times prior thereto employees were exposed to ingestion, inhalation, skin, and eye hazards while using Low VOC PVC cement, primer, and sealant to install a geothermal pvc plumbing system to a water supply, where employees were not trained on the hazards associated with their use.
