(a)  Valencias Roofing @ 5220 Boardwalk Drive, Fort Collins CO 80524:  On and before May 29th 2015, the employer did not ensure employees installing new shingles on a 4 in 12 pitch roof were using fall protection. This condition exposed employees to a fall hazard of approximately 30 feet.    Valencias Roofing was previously cited for a serious violation of the same or substantially similar condition of the Occupational Safety and Health Standard 29 CFR 1926.501 (b) (13); which was contained in OSHA inspection number 1009270, Citation Number 1, Item 2 issued on November 26, 2014 with respect to worksite at 344 Shawnee Lane, Superior CO 80027 and became Final Order on December 9, 2014.
