OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being struck by fragments of Polyvinyl Chloride (PVC) plastic pipe in the event that the PVC plastic pipe ruptured:    a)Wood processing area; On or about June 1, 2015 and at times prior, the employer exposed his employees to the hazard of being struck by fragments of Polyvinyl Chloride (PVC) plastic pipe in the event that the PVC plastic pipe ruptured. PVC plastic piping was used in exposed above ground locations to transport compressed air at 120 psi. The employer failed to follow ASME Gas Transmission and Distribution Piping Systems - ASME Code for Pressure Piping, B31. Section 842.3.3 Installation Provisions which prohibits the above ground use of PVC pipe for compressed air conveyance.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    a) Unimart 23 machine; on June 4, 2015 and times prior the employer exposed employees to caught in and amputation hazards, where the 16 inch, red shaft that drives the exit belt, while removing boards from machine standing within 48 inches .

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:    a) Loading dock area; on June 1, 2015 and times prior the employer exposed employees to explosion and struck by hazards due to allowing oxygen and acetylene gas cylinders to be stored unsecured by the loading dock door. The cylinders were in storage with regulators and hoses attached, but were not secured from falling over or being struck by, but not limited to, employees, forklift, or lumber.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use:  a) Loading dock area; on June 1, 2015 and times prior the employer exposed employees to explosion and struck by hazards due to allowing oxygen and acetylene gas cylinders to be stored beside the loading dock door. The cylinders had the regulators and hoses attached instead of valve protection caps that protected the valves from damage due to falling over or being struck by, but not limited to, employees, forklift, or lumber.
