29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws: On or about June 02, 2015 and at times prior thereto, employees working at a residential building were using an extension cord with visible strain relief damage. Employees were exposed to electrical shock hazards.

Employees were exposed to fall hazards of more than six feet to the ground below.
