29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:    (a) Edger operator - On or about May 20, 2015 employees were exposed to continuous noise at 367% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of 99.4 during the 420 minute sampling period. Exposure calculation included a zero increment for the time not sampled.     (a) Chipper operator - On or about May 20, 2015 employees were exposed to continuous noise at 97.4% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of 99.4 during the 422 minute sampling period. Exposure calculation included a zero increment for the time not sampled.     (a) Scragg mill operator - On or about May 20, 2015 employees were exposed to continuous noise at 204% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of 95.1 during the 420 minute sampling period. Exposure calculation included a zero increment for the time not sampled.     (a) Resaw operator - On or about May 20, 2015 employees were exposed to continuous noise at 195% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of 94.9 during the 425 minute sampling period. Exposure calculation included a zero increment for the time not sampled.

29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position:      (a) Sgagg Mill - On or about April 22, 2015 an employee was un-jamming the edger. The machine was turned off but not locked out. An employee climbed onto the edger started to fall and caught the guard on the motor resulting in an amputation to a finger.
