29 CFR 1910.23(a)(9): Each floor hole, into which persons could not accidentally walk, was not protected by a cover that leaves no openings more than 1 inch wide:  a) #5 Line - There is a 4" wide by 12" long by 6" deep floor opening with a hose in it near the #5 line that was not guarded, thereby exposing the operator to a trip hazard, observed on or about 6-02-15.  b) #9 Line - There is a 4" wide by 13" long by approximately 6" deep floor opening with a hose in it between line 9 and 10 that was not guarded, thereby exposing the employees in the area to a trip hazard, observed on or about 6-02-15.  Abatement certification required.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  (a) Production Floor - Newly hired machine operators did not receive GHS training before being allowed to work on the production floor operating the equipment on the extrusion lines and other equipment. The employer did not provide these employees with the required OSHA Globally Harmonized System of Classification and Labeling of Chemicals (GHS) training prior to the effective date of December 1, 2013, reported on or about 6-2-15.   Abatement certification required.
