29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:  (a) Maintenance Shop: On or about May 5, 2015 and at times prior, employees were exposed to tripping and impalement hazards from piles of cubicles walls, wood pallets and other building materials on the floor, fluorescent light bulbs in a metal trash can, four 5 gallon type II gas cans, a 5 gallon plastic diesel can and other flammables material blocking paths while working in the Maintenance Shop.

THE DEPARTMENT OF DEPARTMENT OF HEALTH AND HUMAN SERVICES, INDIAN HEALTH SERVICE WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315491951, CITATION 01, ITEM NUMBER 005a AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 3, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT HIGHWAY 191 HOSPITAL ROAD, BUILDING 13, CHINLE, ARIZONA 86503.
