29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     a) 2650 Old Pricetown Road, Temple PA; An employee was not using fall protection while standing on top of a stack of 7/16th inch OSB plywood that was on a scaffold platform attached to a Pettibone Model Number 8044 elevated 29 feet 1 inch above the ground, on or about 06/03/2015.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) 2650 Old Pricetown Road, Temple PA; Employees were exposed to a fall ranging in height between a 17 feet 7 inches and 29 feet 1 inch while installing plywood sheeting onto roof trusses without the use of fall protection, on or about 06/03/2015.       Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
