29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) 7 Meadow Ridge Road, Temple, PA; Employees were cutting plywood, and retrieving plywood and on a scaffold platform attached to Pettibone Model Number 8044 that was not attached to the forks in order to prevent horizontal movement, on or about 06/03/2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
