(a) 50-52 W. Houston Ave, Montgomery, PA: Employees were working in an excavation where water was accumulating from a rainstorm and from an existing 12 inch sewer pipe that was actively flowing sewage and no precautions were taken to protect the employees against an engulfment hazard created by the loose soil and water, observed on or about June 2, 2015.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) 50-52 W Houston Ave, Montgomery, PA: Employees working in an excavation 14'5 feet in length and 8 foot in depth were did not have a ladder or other safe means of egress from the excavation, observed on or about June 2, 2015. Anrich, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(c)(20) which was contained in OSHA inspection number 907963, citation number 1, item number 1 and was affirmed as a final order on October 16, 2013 , with respect to a workplace located at W. County Line Rd, near Hidden Woods Lane, Hatboro, PA.
