29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:   a)  Roof - On or about 06/03/2015, a swing stage suspended scaffold was not erected in accordance with the manufacturer's requirements by a supervisor who was considered by his employer to be the site competent person, in that:   1)    when using stirrups to support the scaffolds' outriggers, the rear stirrup was not placed at the end of the outrigger and attached to the outrigger with a pin; 2)    supports were not installed on a 45 degree angle between the stirrups and the bottom of the outrigger.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
