29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    (a) Production area- An employee operating CNC router #2 was exposed to continuous noise at 67.5 % of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 87.2 dBA during the 406 minute sampling period on August 6, 2015. The exposure calculation included a zero increment for the 74 minutes not sampled.     (b) Crate building area- An employee operating a chop saw to cut wood and using a nail gun while building shipping crates was exposed to continuous noise at 78.5 % of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 88.3 dBA during the 402 minute sampling period on August 6, 2015. The exposure calculation included a zero increment for the 78 minutes not sampled.     (c) Production area- An employee cutting metal with the chop saw located closest to the aisleway was exposed to continuous noise at 87.3 % of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 89.0 dBA during the 399 minute sampling period on August 6, 2015. The exposure calculation included a zero increment for the 81 minutes not sampled.     (d) Production area- An employee cutting metal with the chop saw located on the crate side was exposed to continuous noise at 119.8.% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 91.3 dBA during the 396 minute sampling period on August 6, 2015. The exposure calculation included a zero increment for the 84 minutes not sampled.       An effective hearing conservation program would include, initial baseline and annual audiograms, making hearing protectors available, and initial and annual noise training.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (a) Production area - Employees using particulate respirators (dust masks) while cutting wood and/or trimming the edges of the exterior building cladding were not given the information contained in Appendix D of the respirator standard. The dust masks were used on a voluntary basis.
