29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about June 4, 2015, at Lot 41, 2249 Arlington Walk Lane, Grayson, Georgia:  Three employees performing framing work were exposed to struck-by hazards from falling nail guns and lumber of the employees working above them when they worked without using head protection.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about June 4, 2015, at Lot 41, 2249 Arlington Walk Lane, Grayson, Georgia:  a) Two employees performing framing work were exposed to struck-by hazards from wood chips and saw dust when they used two Makita 5007F circular saws without wearing eye protection.  b) Three employees performing framing work were exposed to struck-by hazards from nails and wood chips when they used nail guns without wearing eye protection.

29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  On or about June 4, 2015, at Lot 41, 2249 Arlington Walk Lane, Grayson, Georgia:  Two employees performing framing work were exposed to amputation hazard when they used two Makita 5007F circular saws that had pieces of wood wedged against the saw blades that prevented the lower blade guard from engaging.
