29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Umanzor Framing LLC at 3127 W Conejos Pl, Denver, CO: On or before May 28, 2015 the exposing employer did not ensure that employees were protected from injury, in that two employees framing a section on the top level of a multi-unit residential building were not utilizing a fall protection system. This condition exposed the employees to a fall of approximately 40 feet to the ground on the West side of the building. (b) Umanzor Framing LLC at 3127 W Conejos Pl, Denver, CO: On or before May 28, 2015 the exposing employer did not ensure that employees were protected from injury, in that two employees framing a section on the top level of a multi-unit residential building were not utilizing a fall protection system. This condition exposed the employees to a fall of approximately 10 feet to the level below on the East side of the building. (c) Umanzor Framing LLC at 3127 W Conejos Pl, Denver, CO: On or before May 28, 2015 the exposing employer did not ensure that employees were protected from injury, in that two employees framing a section on the top level of a multi-unit residential building were not utilizing a fall protection system. This condition exposed the employees to a fall of approximately 10 feet by falling through floor joists to the level below. (d) Umanzor Framing LLC at 3127 W Conejos Pl, Denver, CO: On or before May 28, 2015 the exposing employer did not ensure that employees were protected from injury, in that two employees framing on the third level of a multi-unit residential building were not utilizing a fall protection system. This condition exposed the employees to a fall of approximately 30 feet to the ground on the East side of the building.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet above the upper landing, and was not secured: (a) Umanzor Framing LLC at 3127 W Conejos Pl, Denver, CO: On or before May 28, 2015 the exposing employer did not ensure that an aluminum portable extension ladder, which was used to access the top level of a multi-unit residential building, had side rails extending 3 feet above the upper landing, and was secured. This condition exposed two employees to a fall hazard of approximately 10 feet.
