29 CFR  1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On and about October 7, 2021,  employees engaged in construction activities were not protected from fall hazards by conventional fall protection and/or safety devices  (e.g.: guardrail systems, safety net system, or personal fall arrest system).   An employee in the process of installinga framed wall section(s) of a residential property located at 350 N. Milwaukee Street in Denver, CO stepped backwards and fell approximately 30 feet  through an open stairwell shaft  on the second floor of the structure to the basement below.

29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  On and about October 7, 2021, employees in the process of completing the framework for several residential properties were not protected from falling hazards by a personal fall arrest systems, covers, or guardrail systems erected around such holes.  An employee in the process of installing the  framed wall sections of a residential property located at 350 N. Milwaukee Street in Denver, CO stepped backwards, and fell approximately 30 feet  through an open stairwell shaft to the concrete basement below.

29 CFR  1926.501(c)(1): When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels:  On and about October 7, 2021, employees were not protected from struck by hazards in that toe-board used to prevent objects such as, but not limited to, powered tools and/or other materials used during construction activities were not installed.

Employees did not know how to use and/or operate guardrails and the fall protection harnesses.
