29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph. a- 1st Floor sub-panel - On or about May 1, 2015 employees were working in and around a 120/208 VAC electrical conductor that entered a panel and did not use a box connector to prevent strain on the wire and connection, which required the panel cover to be removed, exposing the workers to unguarded live parts.
