29 CFR 1910.253(e)(5)(iv): Hose connections were not clamped or otherwise securely fastened in a manner that would withstand, without leakage, twice the pressure to which they would normally be subjected in service, but in no case less than a pressure of 300 psi (2.04 MPa).       A) Fabrication shop by column B- 2, on or about June 5, 2015, an oxygen hose connected to an oxygen cylinder leaked oxygen when the hose was pressurized, exposing employees to a fire hazard.
