29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from the recognized hazard of damaged insulation: a) Shop; On or about June 3, 2015 and at times prior thereto, the employer permitted employees to use the foot pedal of the HTC Hydra-Tool shear that had damaged insulation on its cord and had been repaired with electrical tape. b) Shop; On or about June 3, 2015 and at times prior thereto, the employer permitted employees to use the foot pedal of the Pacific press break that had damaged insulation on its cord where bare wiring was present.
