29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a)  Pro-Pak Industries, Inc. on or about June 2, 2015, at the address of 1125 Ford St. Maumee, OH: The employer failed to ensure that energy control procedures were utilized to control potentially hazardous energy sources while an employee performed tensioning and alignment adjustments to the feeding rollers of the EMBA 160 Machine. The employee performing this activity in an energized condition was exposed to incoming nip points at and in between the machines feed rollers.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:  a)  Pro-Pak Industries, Inc. on or about June 2, 2015, at the address of 1125 Ford St. Maumee, OH: The employer failed to ensure that energy isolating procedures for shutdown were performed prior to performing tensioning and alignment adjustments to the feeding rollers of the EMBA 160 Machine. An employee performing this task in energized condition was exposed to amputation hazards at the machines point of operation, in that energy control shutdown procedures required under 29 CRF 1910.147(d)(2) were not utilized before the employee began the aforementioned work task on the machine.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    a) Pro-Pak Industries, Inc. on or about June 2, 2015, at the address of 1125 Ford St. Maumee, OH: The employer failed to ensure that employees who are exposed to energized rotating parts during service work on the EMBA 160 Machine were adequately trained in the skills required for the safe application, usage, methods, and means to isolate and control energy sources. The employees tasked with service work on the aforementioned machine were not specifically trained to lockout the machine to protect against amputation injuries prior to performing roller adjustment near the point of rotation.
