29 CFR  1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) Pro-Pak Industries Inc. on or about March 6, 2025, at the address of 1125 Ford St. Maumee, OH: The employer did not ensure employees performing lockout/tagout covered tasks were adequatly trained on means and methods to isolate hazardous energy sources prior to the performance of changing slotter head blades, repairing conveyor belts and clearing jams on the counter ejector of the Jumbo Machine. Without the benefit of this training, employees were exposed to caught in/amputation hazards.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   a) Pro-Pak Industries, Inc. on or about March 3, 2025, at the address of 1125 Ford St. Maumee, OH: The employer did not ensure that energy isolating procedures for shutdown were performed prior to unjamming the counter ejector on the Ward Jumbo Machine. Employees performing these tasks in energized condition were exposed to amputation hazards at or near the counter ejector (between roller nip-points), in that energy control shutdown procedures under 29 CFR 1910.147(d) were not utilized before the employee began the service task on the machine.  b) Pro-Pak Industries, Inc. on or about March 3, 2025, at the address of 1125 Ford St. Maumee, OH: The employer did not ensure that energy isolating procedures for shutdown were performed prior to changing knives on the Ward Jumbo Machine. Employees performing these tasks in energized condition were exposed to amputation hazards at or near the five blade spinners, in that energy control shutdown procedures under 29 CFR 1910.147(d) were not utilized before the employee began the maintenance task on the machine.  c) Pro-Pak Industries, Inc. on or about May 12, 2025, at the address of 1125 Ford St. Maumee, OH: The employer did not ensure that energy isolating procedures for shutdown were performed prior to repairing a conveyor belt on the Ward Jumbo Machine. Two employees tasked with replacing the belt with rings and wire, while the 3rd employee stretched the belt in an energized condition were exposed to amputation hazards at or near nip points in that energy control shutdown procedures under 29 CFR 1910.147(d) were not utilized before the performance of this work on the machine.
