29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     On June 9, 2015, employees working in a trench were exposed to struck-by hazards from excavated material that was located near the edge of the trench and the employer did not ensure employees were utilizing head protection.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

The employer did not ensure that a support system or another method was used to protect employees from the possible collapse of the structure. On June 9, 2015, employees performing work activities in a trench were exposed to a struck-by hazard from excavated material that was at the edge of the trench and the employer did not ensure all excavated material was kept two (2) feet from the edge of the trench.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
