29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): a) On August 3, 2017, employees were not protected from the hazard of cave-in while working in a trench measuring approximately 8.3 feet in depth. Pecora Excavating Contractors, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1069125, citation number 1, item number 5 and was affirmed as a final order on July 23, 2015, with respect to a workplace located at 9 Chatham, Oak Brook, IL 60521.
