29 CFR 1910.106(e)(6)(i): Precautions shall be taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat. a) Fluid Draining Building: On or about June 5, 2015, employees were using an LPG powered industrial truck to move a vehicle with a fuel tank containing gasoline when the fork punctured the fuel tank of the vehicle. When the tank was ruptured, fugitive gasoline vapor was ignited by the powered industrial truck, resulting in a fire within the facility.

29 CFR 1910.178(c)(2)(iv): Power-operated industrial trucks designated as DY, EE, or EX were not used in locations where volatile flammable liquids or flammable gases were handled, processed or used, but in which the hazardous liquids, vapors or gases would normally be confined within closed containers or closed systems where they would escape only in case of accidental rupture or breakdown of such containers or systems. a) Fluid Draining Building: On or about June 5, 2015, An employee was using a standard LPG powered industrial truck to move a vehicle with a fuel tank containing gasoline and punctured the tank with the fork. When the tank was ruptured, the fugitive gasoline vapor was ignited by the powered industrial truck's unmitigated hot components, resulting in vapor ignition and fire within the facility.
