29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency: a) Front of building - On or about May 12, 2015, an employee who had received prior scaffold safety training was working from an articulating boom aerial lift, at a working height approximately 30 feet above grade, wearing a body belt/personal fall arrest harness the lanyard was not attached to the boom or basket, but had not received remedial training to ensure his proficiency.
