29 CFR 1910.176(b): Storage of material created a hazard: a. in the warehouse, steel coils, for use in the chopper machines, were not stored to prevent tipping; coils were moved by hand from where set on the floor to the loading equipment without a lifting device or method to prevent tipping. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a. A non-factory manufactured lifting attachment was used on the Big Joe powered lift, without determination of impact on the maximum lifting capacity of the lift. There was no marking on the lift showing the capacity of a lift with the attachment. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
