OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard:    a/  Inside of the production department, operators of the the unguarded, Maren Engineering Corporation Vertical Baler, m/n 2048, s/n 3197, were not instructed in the safe work methods necessary to cycle this machine safely.  Records were not maintained indicating the name of the employee or the date when this training occurred.    One method of abatement to correct this apparent hazard is to perform effective operator training as required by ANSI 245.5-2004, paragraph 7.1(b). Inside of the production department, rotating parts belonging to the following machines were not guarded:    a/  Four feeders supplying material to the COE Press Equipment, m/n CPR-2512.  b/ Three feeders supplying material to the Rapid Air Corporation Machines, m/n RA1506, s/n 126478.  c/ One feeder supplying material to the DC Motor Speed Control Machine, m/n 7-22177000.   d/ One feeder supplying material to the Rapid Air Corporation Machine, m/n RA4012, s/n 126315.  e/ Two feeders supplying material to the Press Room Equipment Corporation Machines, m/n FF124000413, s/n 0688-033.  f/  Three feeders supplying material to the Magnetek Machines, m/n 4640535143-DA.  g/  Four Core Former Machines, unknow m/n or s/n.    Inside of the production department, in-going nip points belonging to the following machines were not guarded:    a/  Vertical Belt Sander, Home made.  b/  Six Manual Winder Machines.     Inside of the production department, the unused portion of the blade belonging to the MSC Industrial Supply Co. Horizontal Band Saw, No. 942796, was unguarded.

One of these machines caused severe injury to an employee's fingers on or about 04/24/12.    ELECTRO-CORE INC. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii) which was contained in the OSHA Inspection 315430348, citation 1, item 1, which was issued on 03/23/11 and closed on 04/06/11.

29 CFR 1904.32(a)(1):     A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year.    The employer did not ensure that the OSHA 300 and 300A forms were accurate and complete for calendar year 2011 as follows:    1.)  Column A, case number column, was not completed for each injury listed on Form 300.  2.)  Column H, Days Away Injury, was not checked for case number 17303 on Form 300; column I was checked instead.  3.)  The Number of Cases Field H listed 0 instead of 1 for Form 300A.  4.)  The Number of Cases Field I listed 0 instead of 3 for Form 300A  5.)  The Number of Days Away from Work, Field K, listed 0 instead of 13 for Form 300A.  6.)  The Number of Transfer/Restricted Days, Field L, listed 0 instead of 119 for Form 300A.  7.)  The Number of Injuries, Field M(1), listed 0 instead of 7 for Form 300A.  8.)  The Number of Injuries, Field M(3), listed 0 instead of 1 for Form 300A.    ABATEMENT CERTIFICATION AND ABATEMENT VERIFICATION ARE REQUIRED FOR THIS VIOLATION.
