29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On or about 6/10/2015, on the second floor of the mill, the employer allowed employees to walk on beams in order to reach process equipment at an elevation over four feet above the lower level and did not provide railings or any other form of guarding to prevent a fall. Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.

This violation occurred on or about 8/26/2015 when employees were exposed to sound levels above eighty-five (85) dBA while working in the area outside of the control room on the second floor of the mill.
