Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to crushing and struck by injuries:   While performing a site specific targeting inspection walkthrough from June 8 through June 12, 2015 at the Wyman Gordon facility in North Grafton, Massachusetts, four employees were observed operating powered industrial trucks without wearing seat belts exposing them to serious injuries in the event of a tip-over or other accident.

The eyewash in the pump room did not have sufficient pressure to clear an employee's eyes of chemicals in the event of an accidental exposure.

Dimensional inspectors, by the Marvel Saws, were using a cart with a relocatable power tap which was powered via an extension cord.      b. Photo shop employees were using testing equipment powered by a series of extension cords and relocatable power taps.      c. Spray booth, relocatable power tap was used to power pumps.
