29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), shall not be dispensed into containers unless the nozzle and container are electrically interconnected. Where the metallic floorplate on which the container stands while filling is electrically connected to the fill stem or where the fill stem is bonded to the container during filling operations by means of a bond wire:   (a) Spray booth: On or about June 08, 2015, and at times prior, employees were exposed to fire and explosion hazards from flammable liquids such as but not limited to, Lacquer Thinner (Flash point -4 DEG F), Tufcote (Flash point 83 DEG F), OMNI MR 186 (Flash point 24 DEG F), when these flammable liquids were transferred without grounding and bonding.                                                    (b) Prep room: On or about June 08, 2015, and at times prior, employees were exposed to fire and explosion hazards from flammable liquids such as but not limited to, Lacquer Thinner (Flash point -4 DEG F), Tufcote (Flash point 83 DEG F), OMNI MR 186 (Flash point 24 DEG F), when flammable liquids were transferred without grounding and bonding.

29 CFR 1910.107(c)(6): Electrical wiring and equipment not subject to deposits of combustible residues but located in a spraying area as herein defined were not explosion-proof type approved for Class I, group D locations:  (a) Spray booth: On or about June 08, 2015 and at times prior, employees were exposed to fire and explosion hazards when non explosion proof flexible extension cords were used in the spray booth to energize electrical equipment.        (b) Spray booth: On or about June 08, 2015 and at times prior, employees were exposed to fire and explosion hazards when a non explosion proof radio was used in the spray booth.                                   (c) Spray booth: On or about June 08, 2015 and at times prior, employees were exposed to fire and explosion hazards when non explosion lights were used.        (d) Spray booth: On or about June 08, 2015 and at times prior, employees were exposed to fire and explosion hazards when non explosion proof outlets were used in the spray booth.

29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use:   (a) Spray booth: On or about June 08, 2015 and at times prior, employees were exposed to respiratory hazards while using 3M 5300 negative pressure, half face piece respirators in spray painting applications                                                                             (b) Abrasive blasting area: On or about June 08, 2015 and at times prior, employees were exposed to respiratory hazards while using a Bullard supplied air respirator during abrasive blasting operations.
