29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about June 11, 2015, at the above addressed jobsite, to portable ladders used to access the working level did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to fall hazards of twenty (20) feet.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  On or about June 11, 2015, at the above addressed jobsite, a portable ladder with a broken rung was used to access the working level, thereby exposing employees to fall hazards of one (1) foot.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about June 11, 2015, at the above addressed jobsite, employees engaged in residential roofing operations were not protected from falling, exposing employees to fall hazards of twenty-one (21) feet eight (8) inches.   Mauricio Gonzalez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 961396, citation number 1, item number 5 and was affirmed as a final order on April 14, 2014, with respect to a workplace located at 223 S. Bodin St., Hinsdale, IL 60521.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
