Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to thermal burns and the employer did not ensure that steam pipes were covered to prevent employees from making contact with the bare pipes:   Outside of Building #2:  On or about June 8, 2015, an employee was exposed to a struck-by hazard while operating a powered industrial truck, JBL Model 8040, without a seatbelt.

29 CFR 1926.451(c)(2)(v):  The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  Outside of Building #2:  On or about June 8, 2015, employees were exposed to a fall hazard of greater than 10 feet, while working from a platform being elevated by a powered industrial truck, JBL Model 8040, that was not secured to the lifting device.
